New Patient Forms
What to Expect
During your initial visit you can expect:
New Patient Forms: Completing the New Patient Form attached prior to visiting the practice will save approximately 15-20 minutes, and allow you to complete the documents in the comfort of your own home.
Consultation: During the initial health consultation we will discuss your current well-being and review any questions you may have regarding your health and chiropractic wellness.
X-Rays/MRI: A series of X-rays or potentially an MRI may be needed to allow us to clearly determine a proper care plan (this will be determined by spinal examination). Spinal X-rays provide us with an objective analytical tool that will show us the quality of the spine and safety of adjusting it. We also use X-rays to determine how long a subluxation has been developing.
Report of Findings: During your Report of Findings we will share the results that we found with you. You may invite anyone in your family to accompany you, if you choose. We review your current health and discuss a plan of action to help you reach your God given innate potential.
